ALL REGISGTERED PLAYERS AND CHEERLEADERS:

Paperwork Turn-in is set for Wednesday, July 12 from 5:30-7:30 p.m. at the Bellingham Public Library!

If your online registration is completed please log in to your account to print your documents and finalize the registration process for your player.

Click on the Green LOG IN button on the upper right hand corner of the website.  Enter your email address and password that you used to register your player.  Then, your player's name will appear and you can click on PRINT FORMS and choose the documents you'd like to print.  All documents that are in blue are required with the exception of Resume Participation Medical Clearance & Absentee Form.  Please Print All Other Forms!

Listed below are the forms that are required for each player:

  1. Participation, Tracking & ID Card (page 1)
    1. Cheerleaders must upload a photo to the ID card.  Football players will have their photos taken with uniform numbers at the PPK fundraiser event.
  2. Participation Contract, Tracking & ID Card (page 2)
  3. Medical Clearance Form or a Physical Form from your doctor dated in 2017
  4. Waiver & Release of Liability-Minor
  5. MTBI/Concussion Annual Statement & Acknowledgement Form
  6. Emergency Medical Treatment, Consent & Information
  7. 2017 AYF Code of Conduct Form
  8. Image Release-Minor
  9. Report Card - a copy of the participants 2017 year end report card
  10. Proof of Age - "NEW PLAYERS ONLY" - Original Birth Certificate, State/City/Town Raised Seal Certified Copy of a Birth Certificate or a Passport must be provided.  

Please be sure to print/sign all your documents and verify them for accuracy.  All documents should be mail or dropped off by July 1st to:
